What Is Cardiac Catheterization?
Cardiac catheterization is a minimally invasive procedure in which a thin, flexible tube (catheter) is inserted into a blood vessel — typically in the wrist (radial access) or groin (femoral access) — and guided to the heart under X-ray fluoroscopy. Once positioned, the catheter can be used to inject contrast dye, measure pressures within heart chambers, assess valve function, and evaluate the coronary arteries for disease.
The procedure is typically performed under local anesthesia with mild sedation. Most patients go home the same day or within 24 hours. Serious complications are rare in experienced hands.
Coronary Angiography
Coronary angiography — performed as part of the cardiac catheterization procedure — provides direct visualization of the coronary arteries. Contrast dye is injected through the catheter into each artery, producing detailed X-ray images that reveal the location, severity, and extent of any blockages (stenoses).
This information is essential for determining the most appropriate treatment strategy:
- Mild to moderate disease may be managed medically with medications and lifestyle modification
- Significant single or multi-vessel disease may be best treated with percutaneous coronary intervention (PCI) with stent placement
- Complex or multi-vessel disease may be referred for coronary artery bypass grafting (CABG)
Percutaneous Coronary Intervention (PCI) and Stenting
When coronary angiography reveals a significant blockage, PCI — commonly called coronary angioplasty — can often be performed in the same session. During PCI:
- A small balloon is advanced to the site of the blockage and briefly inflated to compress the plaque and widen the artery
- In nearly all cases, a coronary stent — a small metal mesh tube — is deployed at the site to maintain the opening and reduce the risk of re-narrowing (restenosis)
- Modern drug-eluting stents are coated with medication that further reduces the risk of restenosis over time
For patients presenting with an acute heart attack (STEMI), emergency PCI — known as primary PCI — is the most effective treatment to restore blood flow and preserve heart muscle.

Right Heart Catheterization
Used to measure pressures within the right heart chambers and pulmonary arteries, this procedure is essential for evaluating heart failure, pulmonary hypertension, and valvular disease.
Structural Heart Interventions
Catheter-based techniques are increasingly used to treat structural heart conditions without open surgery, including transcatheter aortic valve replacement (TAVR) for aortic stenosis and MitraClip for mitral regurgitation.
When Is Cardiac Catheterization Recommended?
Your cardiologist may recommend cardiac catheterization if you have:
- Abnormal findings on non-invasive stress testing or coronary CT angiography
- Unstable angina or symptoms of acute coronary syndrome
- Symptoms consistent with significant valvular disease
- Unexplained heart failure requiring hemodynamic assessment
- Prior to major cardiac or non-cardiac surgery in high-risk patients
