What Is a Carotid Doppler Ultrasound?
Carotid duplex ultrasound combines two complementary imaging techniques:
- B-mode (brightness mode) imaging: produces real-time, two-dimensional images of the carotid artery walls, identifying plaque, wall thickening (intima-media thickness), and calcification
- Doppler flow analysis: measures the velocity and direction of blood flow through the vessels, with elevated velocities indicating narrowing (stenosis)
Together, these techniques provide a comprehensive assessment of both carotid artery structure and function. The study is painless and typically completed in 30–45 minutes.
What Can a Carotid Doppler Detect?
Carotid Artery Stenosis
Plaque buildup in the carotid arteries can progressively narrow the vessel — a condition called stenosis. Flow velocities measured by Doppler allow precise quantification of the degree of stenosis, which guides treatment decisions:
- Moderate stenosis (50–69%) is typically managed medically with antiplatelet agents, statins, and blood pressure control
- Severe stenosis (70% or greater) may warrant surgical intervention (carotid endarterectomy) or catheter-based stenting, particularly in symptomatic patients
Carotid Plaque Characterization
Not all plaque carries the same risk. Soft, echolucent (lipid-rich) plaque is more prone to rupture and embolism than hard, calcified plaque. Carotid duplex allows assessment of plaque morphology, which contributes to individual stroke risk assessment.
Intima-Media Thickness (IMT)
Measurement of the carotid intima-media thickness is a marker of subclinical atherosclerosis and cardiovascular risk. Elevated IMT has been associated with increased risk of future heart attack and stroke, and may influence the decision to initiate preventive therapies.
When Is a Carotid Doppler Ordered?
A carotid duplex ultrasound is recommended for patients with:
- Sudden neurological symptoms: weakness, numbness, speech difficulty, or vision loss (possible TIA or stroke)
- A carotid bruit (abnormal sound detected on physical examination)
- Known atherosclerotic disease in other vascular beds (coronary, peripheral)
- Multiple stroke risk factors: hypertension, diabetes, hyperlipidemia, smoking, family history
- Pre-operative screening in high-risk patients undergoing major cardiac or vascular surgery
- Monitoring of previously identified carotid stenosis
What to Expect During Your Study
You will lie on an examination table while a gel is applied to the sides of your neck. Our trained vascular sonographer will gently move the transducer along the carotid arteries, capturing images and flow measurements. The study is entirely painless, requires no special preparation, and delivers no radiation. Results are reviewed by a board-certified physician and discussed with you promptly.
